Referrals can be made for any of these services by individuals, physicians, educators, lawyers, and other health professionals.

The Cypress Health Region Community Health Services programs are located at the E.I. Wood Building in Swift Current.

You may access your personal immunization records through community health by completing one of the forms below and calling (306) 778-5280.  Please be advised that a fee may be associated with accessing your immunization records.
